After making your appointment, please save a patient form.

With newest Internet Browser 7, (also works with IE6}
RIGHT click Download above -
click
"Save Target As..."

or in older browsers,
Click Download above.
In the new window that appears, if
the choice is between Open & Save,
click Save. If the document opens instead,
click the Save button.
You must name the file, eg. Adult.pdf

Print form, fill it out & fax back to the office,
before your scheduled appointment.
Please remember to bring in copies of any recent lab work
or medical records as well as the supplements
or medications you are currently taking.
